Please remember if you are booking an appointment, that is for in-store service only. If you are RSVP-ing to an event, only RSVP to an event for the community that you live in. We will cancel appointments for bookings that are made by folks whose home address does not align with the community the appointment is booked in to make room for persons that do live in the community. Events have limited space/inventory. If the event is full, we will not have extra inventory for walk-ins.
Please be aware if your insurance does not cover your vaccine you will be financially responsible. If you are unsure of your coverage you must check with your insurance company PRIOR to receiving the vaccine.

Covid Testing:
The forms we use in the store will be handwritten. If you need the form typed fill it out here and print it in advance and bring it with you. Make sure you make the dates match the dates of your test appointment. We can not print this form from your computer in the store. You must print it yourself.
