-Referral Source must be existing and regular current customer.
-Max 4 referrals per person.
-One per household.
-Family members living in the same household do not qualify.
-Not applicable to external discount drug cards.
-New customer must transfer entire prescription drug monthly medication profile to qualify.
-Gift cards will be activated and issued to the person(s) in store only and do not expire.
-Gift cards cannot be used for copays.
